IMPORTANT INFORMATION AND CLIENT CONSENT

CLIENT THERAPIST RELATIONSHIP: You and your therapist have a professional relationship existing exclusively for therapeutic treatment. This relationship functions most effectively when it remains strictly professional and involves only the therapeutic aspect. Your therapist can best serve your needs by focusing solely on therapy and avoiding any type of social or business relationship.
RISKS AND BENEFITS: Counseling and art therapy activities related to self-initiating plans are beneficial, but as with any treatment, there are inherent risks. Art therapy activities will help manage psychological concerns. During the activity, you will face questions and situations about personal issues which may bring to the surface uncomfortable emotions such as anger, guilt, confusion, and sadness. The benefits of this can far outweigh any discomfort encountered during the process. Some of the possible benefits are improved personal relationships, reduced feelings of emotional distress, enhanced self-esteem, gaining insight into the problem, and specific problem-solving. We cannot guarantee these benefits, of course. It is our desire, however, to work with you to attain your personal goals for counseling and/or art therapy.
COUNSELING: It is a confidential process designed to help you address your concerns, come to a greater understanding of yourself, and learn effective personal and interpersonal coping strategies. It involves a relationship between you and a trained therapist who has the desire and willingness to help you accomplish your individual goals. Your therapist is available to support you throughout the counseling process. Your first session will be an assessment session in which you and your therapist will determine your concerns, and if both agree that we can meet your therapeutic needs, a plan of treatment will be developed. You can choose not to follow the plan of treatment designed by your therapist, so services can be terminated. The goal of this therapeutic guide is to provide the most effective therapeutic experience available to you. If at any time you feel that you and your current therapist are not a good fit, please discuss this matter with your therapist to determine if transferring to a more suitable therapist is right for you. Wellness is more than the absence of disease; it is a state of optimal well-being. It goes beyond the curing of illness to achieving health. Through the ongoing integration of our physical, emotional, mental, and spiritual selves, each person has the opportunity to create and preserve a whole and happy life. The services are designed to provide our clients with an integrated solution for their mind, body, spirit, and life to enhance their lives and resolve issues.
YOUR RIGHTS: All individuals who are seeking and/or receiving services will be provided with effective, efficient services. These services will be directed toward health and rehabilitation. As an individual receiving services, you have the following rights:

To be treated with consideration and respect for human dignity

To receive quality treatment regardless of race, religion, sex, age, sexuality, ethnic background, and mental and/or physically disabling condition.

To be provided confidentiality and protection from any unwarranted disclosure regarding your treatment.

To be involved in planning your treatment and to be informed about your treatment process.

To be involved in your after-termination follow-up schedule.

To refuse treatment to the extent permitted by law and to be informed of the possible consequences of your actions.

Schedule an appointment with your counselor to review your record and receive any needed explanation about the contents.

If there is evidence of the clear and imminent danger of harm to self and/or others, a therapist is legally required to report this information to the authorities responsible for ensuring safety.
GROUP THERAPY
Group therapy is typically scheduled on a weekly basis and is approximately 4-5 hours long over weekends mostly. More frequent sessions or an intensive schedule are available if determined appropriately by your therapist. If you are unable to attend your a group therapy session, I would ask you to call in advance, whenever possible, and can also help. However, no refund is provided for group therapy. This includes but is not limited to free Sessions such as: Artpsy - Summer Internship, let's paint this.
CONFIDENTIALITY
If your consent has been taken to be a part of the research, I would like to assure you that the research follows all ethical standards prescribed by law. We are required by practice guidelines and standards of care to keep records of your intervention. These records are confidential with the exceptions noted below. Discussions between a therapist and a client are confidential. If you have any questions regarding confidentiality, you should bring them to the attention of the therapist when you and the therapist discuss this matter further. Informed consent will be signed with you before starting therapy, by signing this Information and Consent Form, you are giving consent to the undersigned therapist to keep the records of confidential information for providing your mental health care services and you are also releasing and holding harmless the undersigned therapist from any departure from your right of confidentiality that may result. Furthermore, as a participant, I agree to the following:
This agreement is a contract for confidentiality among the members of the group therapy the purpose of which is the improvement of the mental health of a specific group of individuals through art therapy interventions.
You agree to keep all that happens during the workshop including but not limited to activities conducted, and group therapy discussions where participants share their personal life situations confidential.
Every member of the group acknowledges the need to keep personal information shared in the group private.
Any information shared by a group member the group during group therapy including but not limited to the unity circle about themself should be considered personal and private information.
In order to become a group member and maintain membership, each member must agree to protect this private information. Information gathered about other members of the group cannot be shared with anyone else.
I understand that my pictures and videos will be used on Art Adventures Studios Website and Social Media, however, none of my confidential information will be shared.
For the purpose of research, I will be assigned a participant number so that my name is not visible in the research.
Art Adventures will never share your data such as your name or phone number with any third party.
If any specific information that is necessary for the research is to be used then your consent will be taken by the Art Adventures Studio team.
Art Adventures Studio's facilitator will debrief you about everything related to confidentiality and participation in research on day 1 of the individual or group session.

You agree that we may use personally identifiable information about you to improve our marketing and promotional efforts, analyze site usage, improve our content and product offerings, and customize the Site’s content, layout, and services. These uses improve the site and better tailor it to meet your needs. You agree that we may use your information to contact you and deliver information that, in some cases, is targeted to your interests, such as targeted banner advertisements, administrative notices, product offerings, and communications relevant to your use of the Site.
